New challenges. . The increasing demand for implan $New challenges. . The increasing demand for implants has lead to an increasing amount of implant surgeons, implant companies and designs. . With it comes a new barrage of new dental complications that, with time, we need to learn to deal with. . This nice gentleman had a bicon implant placed UL2 in his late teens/early twenties some 20 years ago. It is hard to know if the implant position is infraoccluded through maxillary growth, or just initial poor surgical positioning. Unfortunately his UL1 has now fractured at gingival level and he is looking for options. . Bicon implants are morse taper friction fit abutments which makes them notoriously difficult to re-restore. There is also no screw channel to engage for reverse torquing, meaning they usually have to be removed with forceps or a trephine; which can be destructive. . There are many treatment plans that could have been implemented here, this is what we did: . 1. Extraction of the UL1 root. 2. Removal of the crown and abutment and sealing of the bicon implant. L-PRF/allograft/fibrinogen ridge augmentation to bury the existing bicon implant. 3. Return after 4 months for implant placement UL1 with crown lengthening UR1. 4. Following a further 4 months healing new crown UR1 with implant bridge UL1(2) with a distal cantilever. . It may not perfect (It’s not composite bonding anyway 😉!), but I’m happy as it’s a vast improvement given the initial presentation. . These are scenarios we will continue to see more of as more implants are placed. Each case has specific wants and needs and an appropriate material should be selected on that alone. This maxillary rehabilitation was required due to tooth surface loss caused by parafuction. Therefore our primary concern was strength and fracture resistance. A Zircona material with excellent physical and aesthetic properties was selected.(zirkonzahn Prettau 4 anterior)$

The four maxillary incisors had secondary caries, fractured veneer restorations and significant palatal tooth surface loss. Although this looks like a simple case of four crown restorations a little more thought was required. Due to the lack of tooth structure palatal restorative space had to be sought. Using four lab made provisional crowns with the Dahl effect incorporated, we gained restorative space over six months. We were then in the ideal position to restore the four anterior incisors with four emax crowns.$
